Below is a link to download a copy of our client questionnaire in PDF format to help you better prepare for our initial meeting. 

It is not necessary for you to complete this questionnaire in advance as we will be going over everything together anyway.  If you do choose to complete it on your own please do the very best that you can providing the needed information.  Write down questions or concerns that you have on the last page and we will review those and help you to complete the questionnaire at our first meeting.

If you do not have credit reports from all three credit repositories (Equifax, Experian and Transunion) then please go to www.annualcreditreport.com and pull them up and print them.  This is a free web site and you don’t have to join anything.  These credit reports are very important.  If you choose to use a different source for your credit reports that is fine, but DO NOT use “freecreditreport.com”. Their credit reports are not suited for Bankruptcy work.
